Learning about your medical history and current health status is key to helping us provide you with the most effective care possible. Before visiting our office for the first time, please consider completing the following patient forms in advance; you can then bring them with you on your first visit. If you don’t have an opportunity to do so, you’ll also be able to fill them out at our office.

Your First Visit

During your first visit, we’ll want to learn more about you and how we can support your dental health. Depending on your needs, it may involve a comprehensive oral exam, X-rays, teeth cleaning, and a one-on-one consultation with Dr. Van Pelt.
